Feedback on Prototype From IGS

I have attached a document I handed to the three main people I interacted with at IGS in Indore with relation to the handcart prototype. Rather than rate the design it simply asks for design requirements for a design from their perspective first with more multiple choice questions, and then with a 2nd page for them to answer in a free-form fashion.
